There are two new Medicare Learning Network (MLN) articles pertaining to the Patient Driven Grouping Model (PDGM).
MLN SE 19027 Overview of the Patient Driven Groupings Model is intended for physicians who order home health services. This article includes eligibility criteria for Medicare home health services as well as some information on certification in addition to PDGM. The section titled “The Importance of Diagnosis Reporting and Physician Documentation Under the PDGM” goes into more detail about the information physicians will need to provide to home health agencies.
While there is an overview of the PDGM, it is MLN SE 19028 Payments and Payment Adjustments under the PatientDriven Groupings Model that provides a more detailed overall description of the key components of the PDGM.
